SpacerHSA Cost of Living Adjustment (COLA) for 2010 Library
     
 

The IRS publishes Cost of Living Adjustments (COLA) each year based on the Consumer Price Index (CPI) changes issued by the Department of Labor. We make required changes to deductibles, out-of-pocket maximums, and annual contributions of Health Savings Account (HSA) plans, based on the CPI.

The following table shows the changes for HSA plans effective January 1, 2010. HSA Adoption Agreement forms have been revised to include these changes. The 2010 changes have been updated in the HSA Basics presentation in the Agent Library, available at www.assuranthealthsales.com.

Since October 1, your clients have had access to simpler, faster online enrollment for HSA Tools. Due to the change to online enrollment, access to the paper form has been removed from the quoting systems. The enrollment form is still available on Find a Form and through Customer Service.

SpacerHealth Access Plan A changes
     
 

You’ll see the following changes for Health Access Plan A with effective dates of January 1, 2010, and later:

Drug coverage changes

  • Prescription Drug Calendar Year Maximum Benefit:  $200 per calendar year (was $750)
  • Generic Drug Copay:  $20 (was $10)
  • Brand Name Drug Copay:  $75 (was $50)

Note: These changes apply to drug coverage in all states where Health Access Plan A is available except Arizona and Tennessee.

New rates
There are new rates for Plan A with effective dates of January 1, 2010, and later in all states except Delaware, Indiana, Mississippi, New Mexico, Ohio, South Dakota and West Virginia. The application packet rate sheets and the online quoting and submission system have been updated. Be sure to get updated application packets for your state.

Dental-Vision Discount Plan is now automatically included with Plan A. The premium for the Dental-Vision Discount Plan is included in the plan rates, except in states where the Discount Plan is not an option: Alaska, Florida, South Dakota and Wyoming.

  • Revised applications with simplified medical questions are available in DE, ID, IN, ME, MO, OK, SD, TN, TX and WV for both Short Term Medical (STM) and HealthSaver® plans. With these changes, even more customers can qualify for temporary health insurance coverage.
  • Short Term Medical and HealthSaver applications have also been updated in KS, NM and WI.
  • Contracts and benefit summaries were updated with required state-specific information in DE, ID, IN, ME, MO, OK, SD, TN, TX and WV.
  • Outlines of coverage were updated in ID, ME, MO, SD, TX and WV.
  • In addition, updated contracts and benefit summaries are available in NM and PA. Immunization language has been updated in the PA contract, and autism and cancer clinical trials benefits have been added to the NM contract. NM also has an updated outline of coverage.

SpacerCoinsurance out-of-pocket maximum changes
     
 

You’ll see changes to the coinsurance out-of-pocket (OOP) maximum amounts on the 12-month plans in AR, DE, ID, KS, MD, ME, MS, NC, UT and WV.

These are the new coinsurance out-of-pocket maximum amounts on individual (one person) plans:

  • 12-month individual Short Term Medical Library plans: The coinsurance OOP maximum will go from $5,000 to $12,500 for 50% coinsurance plans and from $2,000 to $5,000 for 80% coinsurance plans
  • 12-month individual HealthSaver Library plans: The coinsurance OOP maximum will go from $10,000 to $15,000 for 50% coinsurance plans and from $4,000 to $6,000 for 80% coinsurance plans

The product materials and the quoting systems have been updated with the changes.

SpacerSTM software and field-issue changes
     
 

Note: This information applies only to Time Insurance General Agents.

The number of agents who quote and issue Short Term Medical (STM) business online continues to grow. Because of this trend to online sales, we are removing the field-issue functionality from the STM proposal software. Software will continue to be available for download from the Assurant Health Sales Web site to run STM quotes.

This change does not affect the paper STM mailer applications/brochures. You may still submit business using STM paper mailer applications/brochures. And you may field issue online, or quote and issue STM business online at www.assuranthealthsales.com.

There are many good reasons to quote and submit STM business online, including:

  • Always having the most current rates and enrollment information.
  • Eliminating the need for agents to keep and monitor supplies of paper policies/contracts.
  • Giving clients a choice of paper or electronic policy delivery. In most cases, clients who choose electronic delivery receive their policies minutes after submission via e-mail.

Note: This information applies to Time Insurance General Agents. Because of the changes, STM field-issued contracts/policies are not available for order to new agents.

SpacerHIPAA rule changes impact agents Library
     
 

Recently, the Health Insurance Portability and Accountability Act (HIPAA) privacy and security regulations were amended with the enactment of the American Recovery and Reinvestment Act, which includes the Health Information Technology for Economic and Clinical Health Act (HITECH). HITECH has expanded the obligations of business associates, such as our agents. You have always been contractually obligated to comply with HIPAA. As of February 17, 2010, this new law legally requires you to be compliant with HIPAA.

In October, we sent an e-mail to producing agents to further explain the requirements under the new law. Please read the information carefully and keep these two points in mind:

  • If there is a breach of insured information let us know as soon as possible by contacting Judy Titera, Assurant Health Privacy Officer, at judy.titera@assurant.com or at 414-299-1140.
  • Your accountability has increased under HITECH. As of February 17, 2010, you will be legally required to be compliant with HIPAA.

  • System time-out extended to 55 minutes so you can keep working in MyBook following short interruptions
  • New Log Out button for a quick exit

IM Changes:
  • Student Select rate changes: Rate changes will be implemented in the state of California. Use the new brochure for updated information.
  • Agent attestation: Agents who assist their clients in submitting an application must attest on the written application that the information on the application is complete and accurate and that they explained the risk of providing inaccurate information to the applicant.

Colorado

IM Changes:

  • Great-West Healthcare Network (GWH) has changed its name to GWH-CIGNA PPO. Insureds with the GWH network will receive new ID cards.
  • In compliance with the state of Colorado, any plans issued, renewed or reinstated on or after January 1, 2010 will provide coverage for the total cost of covered preventive services. The deductible and coinsurance are waived but applicable copayments may apply. See the Colorado state variations for the list of covered preventive services.

AAHA Changes:

Due to new state mandates, we are no longer offering Health Access
Plan A
in Colorado as of January 1, 2010. The last accepted application signed date is November 30, 2009, and the last accepted effective date is December 15, 2009.

IM Changes:
  • LibraryCoreMed and MaxPlan have simplified rate guarantee options. Your clients can choose a 12-, 24- or 36-month rate guarantee with any deductibles of $2,000 and up.   
  • Library MaxPlan and CoreMed without the office visit copay and OneDeductible new business now include TelaDoc membership, a service that offers physician access — 24 hours a day, 7 days a week, and 365 days a year for adults and children as young as age 10. More information is available in the Agent FAQ J-60987 on Find a Form.
  • LibraryOne Decreasing Deductible is now included with OneDeductible Plans in Kentucky. One Decreasing Deductible rewards your clients with a 10% credit twice a year that reduces the amount they pay of their plan deductible. One Decreasing Deductible is included on OneDeductible 100% coinsurance plans with deductibles at or above $2,850 individual / $5,700 family.     • CoreMed and MaxPlan – the total OON deductible is now two times the network deductible for individual deductibles between $1,500 and $15,000. The same changes impact the corresponding family deductibles and total out-of-pocket amounts.
    • OneDeductible - the total OON deductible is now two times the network deductible for all individual deductibles and family deductibles. In addition, the OON coinsurance has been changed to 50% for both the 80% and 100% network coinsurance plans.

    To comply with state regulations, the OON coinsurance is changing from 30% to 50% on the RightStart and SaveRight policies with 50% coinsurance. The out-of-network stop loss will be increased on these plans to maintain the same out-of-network out-of-pocket maximum. 
  • Great-West Healthcare Network (GWH) has changed its name to GWH-CIGNA PPO. Insureds with the GWH network will receive new ID cards.
  • Effective October 1, 2009, Assurant Health policies provide equal coverage for both opposite-sex couples and same-sex domestic partnerships.

STM Changes:

Assurant Health policies provide coverage equally to individuals in state-registered domestic partnerships and married couples. Quoting systems have been updated.

Texas IM Changes:
  • The out-of-network (OON) benefits have been adjusted for CoreMed, MaxPlan and OneDeductible.
    • CoreMed and MaxPlan – the total OON deductible is now two times the network deductible for individual deductibles between $1,500 and $15,000 and their corresponding family deductibles.
    • OneDeductible - the total OON deductible is now two times the network deductible for all individual deductibles and family deductibles. In addition, the OON coinsurance has been changed to 50% for both the 80% and 100% network coinsurance plans.
  • Good Shepherd Medical Center network is now available in ZIP codes (75451); (75551, 55-56, 60, 62-66, 68, 71-72); 756; (75755, 97). Good Shepherd Medical Center network is priced for introduction in parts of the East Texas market (ZIP codes 755-756). It will be the lowest priced network in the market and offers access to the Good Shepherd Medical Center in Longview.
  • The Austin Regional Independent Association (ARIA) network will be closed to new business effective December 1, 2009. This affects the ARIA Network in Austin (AUT).
  • The Medical Care Referral Group (MCRG) network will be closed to new business effective December 1, 2009. This affects the MCRG Network in El Paso (ESO).
  • Great-West Healthcare Network (GWH) has changed its name to GWH-CIGNA PPO. Insureds with the GWH network will receive new ID cards.
  • In compliance with the state, plans provide coverage for prosthetic devices, orthotic devices and professional services related to the fitting and use of those devices. See the state variations for additional information.

Utah
IM Changes:
  • Great-West Healthcare Network (GWH) has changed its name to GWH-CIGNA PPO. Insureds with the GWH network will receive new ID cards.
  • A new Universal Application Packet is available on Find a Form. The packet includes the Universal Application and all other forms needed when applying for individual medical insurance. The Universal Application is a uniform application form, which is now required by the state of Utah for use by all insurance companies. Applications other than the Universal Application will not be accepted as of January 1, 2010. The completed packet must be used for Individual Medical business submitted on or after January 1, 2010.

    Because of the application changes you can continue to quote using the online quoting system and Software, but you cannot submit applications online.
  • Compensation disclosure: Agents must, at the time of sale or no later than the delivery of the policy, disclose, in writing, that you will be paid a commission, the amount and type known at the time of disclosure. You must keep disclosure records. The Universal Application Packet includes a place where the agent can provide the disclosure. You must obtain and keep an updated record (disclosure and signature) if you receive renewal compensation 36 months after the initial disclosure.

IM Changes:

NovaNet PPO LLC acquired Tennessee Healthcare (THC) effective September 1, 2009. Insureds with the Tennessee Healthcare network will receive new ID cards. NovaNet is closed to new business (includes THC business) effective November 1, 2009.

Washington

IM and STM Changes:

  • Effective November 1, 2009, Assurant Health policies provide coverage equally to individuals in state-registered domestic partnerships and married couples. Quoting systems have been updated.

STM Changes:

  • The state of Washington recently implemented a minimum loss ratio requirement for temporary health insurance. While we never want to take any action that affects compensation, this mandated change is significant. As a result, the agent compensation rate for Washington Short Term Medical new business will be 5 points lower than your current commission level. This change will take place for new business with effective dates of January 1, 2010, and later. If you have questions or would like to discuss, please contact your Sales Manager.

Wisconsin

IM Changes:

As a result of network repricing, Preferred One is now the lowest priced network in northwest Wisconsin.

STM Changes:

New rewrite laws limit consecutive coverage to a total of 18 months. Short term health plans are considered to be consecutive if there are 63 days or fewer between the coverage periods. A question about the total length of coverage is now included on the WI application.

IM Changes:

Great-West Healthcare Network (GWH) has changed its name to GWH-CIGNA PPO. Insureds with the GWH network will receive new ID cards.
